Payment for services is due in cash or check at the time of the service. Significant discounts are available for prepayment of services.

FEE SCHEDULE FOR PHYSICIAN SERVICES

These rates cover Dr. Arthur’s time. Her services offer you the convenience of being treated at home or at work, avoid wasting your time in waiting rooms, playing phone tag, lost hours from work, etc. Many simple problems for the established patient may be handled by e-mail or phone. E-mail and phone consults are not available until you have an initial visit in the office.

For those individuals with multiple medical problems, an in-office visit or home visit for as long as you like is an option. If lab, x-ray or referrals are needed Dr. Arthur will schedule these for you at your convenience.

As a preferred provider in Alabama e-consults, phone visits, nutritional/herb-supplement consultations and myofascial release are not covered by preferred provider organizations, HMOs, or Medicare; therefore Dr. Arthur has not signed on with any medical insurance plans as a preferred provider for this practice. For information regarding your medical insurance coverage of labs, x-ray check, and medications ordered by a nonpreferred provider check with your insurance representative or booklet your insurance company sent you about your plan.

Dr. Arthur is not seeing Medicare patients at this time.
